Our skilled and intermediate nursing facilities provide a high level of nursing and other medical care, as well as personal care and assistance, for people whose illness or impairments require close monitoring.

Around the clock nursing is available with registered nurses on duty at all times. In addition to nursing, other medical services are provided including various rehabilitative therapies: 

- Ventilator Services -

We provide 24 hour respiratory services and are staffed with a full-time respiratory therapist and a consulting pulmonologist for the treatment of ventilator dependent patients. We provide ventilator weaning and long term ventilator management services.

- Suite -

CHCC Facilities offer a Transitional Living Suite for residents who have been in our care and are soon to be discharged. CHCC suite is available for residents with 24 hour care to help along with the transition from CHCC to home. In addition to nursing services, various rehabilitative therapies are available: 

• Physical Therapy
• Occupational Therapy
• Speech-Language Pathology
• Aquatic Therapy (Indoor Therapy Pool)

CHCC's Transitional Living Suite offers: 

• Private room
• Private bathroom with tub/shower
• Access to fully equipped kitchen
• ADL (Assisted Daily Living) transitional assessment
• Emergency response
