Crestview Health Care Center is a 98 bed Skilled Nursing, Subacute and Rehabilitation facility. Our goal is to provide comfortable living to
all who are in our care. Crestview Health Care Center is a contemporary Medicare and Medicaid approved facility specializing in short-term rehabilitation services. The inpatient/outpatient rehab department
includes physical, occupational, speech therapies, indoor aquatic therapy pool, as well as complimentary van transportation for outpatient services.Our licensed professionals provide comprehensive short-term medical and rehabilitative care to persons of all ages. Nearly all patients
return home to continue their rehabilitation. CHCC Home Health Care offers specialized therapeutic services provided by a team of professional including: nurses, home health aides, physical therapists,
occupational therapists and speech – language pathologists. Crestview Health Care also offers respiratory therapy and ventilator care services. An on site dialysis center is available to specialize in renal
care services for patients with kidney disease, including patients suffering from kidney failure.
